Parent/Child Pre-Exam Questionnaire
VisionUnlocked provides an online form designed to collect information regarding your child patient, prior to them coming in for their appointment. This saves time and improves the accuracy of the information provided, as it allows the parent time to think about their answers or even to find the information while still at home.
Following information gathering, you are provided a summary of presenting complaint, medications, with information on use and ocular side effects, patient ocular, medical and scholastic history, family ocular, medical and scholastic history, sports, musical instruments and interests. Where possible we integrate with your practice management software to find or create the patient in your files along with an examination screen with the above details pre-entered.
With this pre-examination details at your fingertips you are saved time every time VisionUnlocked is used. Following your evaluation, where possible, our API draws eye exam information from your practice software and combines this with preliminary data and the results from the iPad vision therapist based assessment to generate a comprehensive vision report. This report comes to you with your practice details and logo promoted on every page.
Adult Pre-Exam Questionnaire
This is another time-saving web-based form which collects patient data, and provides a summary for the optometrist.
The analysis from the questionnaire informs the practice about drugs being used, how the patient uses their eyes, screen time, dry eyes, outdoors activities, headaches, problems with current glasses, queries about contact lenses or surgery and even provides a scaled score on Quality of Life with dry eye. This in turn generates information for increased sales etc.
Reporting and Analysis
A multiple page report following testing is created and emailed directly to the practitioner, ready for discussion with the patient and forwarding to their school. The behavioural optometrist is still in control and can add or modify any of the wording in the report and can contribute to ongoing application changes to improve the effectiveness of the program.
The reporting is available in letter, spreadsheet and graphical format and provides you, your vision therapist and your patient’s parents with the information required to ascertain where a child between the ages of 6 and 14 years may be having vision developmental difficulties which in turn lead to learning difficulties.
